  - Add \todo about a4 not being valid for PAPERSIZE
  - Add PAPERSIZE to BUILD_DEFS
  - Add \todo about jpeg2000 bl3 that is commented out without explanation
  - GC undefined and unused PLIST.cidfmap

No real change intended, other than BUILD_DEFS.  Ride recent
PKGREVISION++.
