Move "INSTALL" to "INSTALL.txt" so the "install" \ 
targets run as expected
on macOS with case-insensitive filesystem, fixing install of cxref.1.

While here, set LICENSE, update MASTER_SITES and HOMEPAGE, and remove
unrecognized configure option "--with-cxref-cpp".
