Handle more RFC 8792 processing:

* Handle RFC8792 unfolding in kramdown-rfc-extract-sourcecode (unless
  --no-unfold)
* Do RFC 8792 extraction before checking JSON in sourcecode blocks
* can use check="skipheader-json" to skip an HTTP header before JSON
  checking
* Handle newline-as-separator bug in RFC 8792
